 NEW  FCC  FORM  301-  CA 
 Application  for  Authority  to  Make  Changes  in  a  Class  A  Television  Broadcast  Station 
 A  new  FCC  Form  301-  CA  (Application  for  Authority  to  Make  Changes  in  Class  A  Television  Broadcast  Station) 
 has  been  approved  by  the  Office  of  Management  and  Budget  and  is  now  effective  and  available. 


 The  FCC  Form  301-  CA  is  to  be  used  by  a  Class  A  TV  licensee  that  seeks  to  make  a  major  or  minor  change  in  its 
 station’s  authorized  facilities.  This  form  is  also  to  be  used  by  a  Class  A  TV  permittee  to  seek  a  major  or  minor 
 modification  of  its  station’s  construction  permit. 


 The  new  FCC  301-  CA  is  available  through  the  Internet.  To  retrieve  the  new  form  by  using  the  Internet,  access 
 the  FCC’s  home  page,  http://  www.  fcc.  gov,  and  click  on  the  Forms  link.  Printed  copies  of  the  FCC  301-  CA  can 
 be  obtained  by  writing  to  the  Commission’s  Form  Distribution  Center,  9300  East  Hampton  Drive,  Capital 
 Heights,  Maryland  20743,  or  by  calling  1  (800)  418-  3676  and  leaving  a  request  on  the  answering  machine 
 provided  for  this  purpose. 


 A  separate  “Remittance  Advice”  form  (FCC  Form  159,  dated  July  1997)  must  also  be  submitted  with  any  FCC 
 Form  301-  CA  requiring  the  filing  of  a  fee.
