 FCC  Form  159  (Remittance  Advice) 
 Effective  for  all  payments  received  by  the  Commission  on  and  after  December  3,  2001,  the  FCC 
 Form  159,  dated  July  1997  will  be  obsolete.  The  latest  version  dated  February  2000  will  be  the  only  acceptable  form. 
 The  February  2000  version  requires  that  you  provide  a  FCC  Registration  Number 
 (FRN)  for  both  applicant(  s)  and  payer.  Those  applications/  filings  received  with  the  obsolete  FCC  Form 
 159  (dated  July  1997)  will  be  returned  as  unprocessable. 


 A  copy  of  the  acceptable  version  is  available  on  the  FCC  website  at  http://  www.  fcc.  gov/  fees.  Click  on 
 "Form  159"  located  on  the  left  side  of  the  page  and  you  will  be  able  to  either  print  a  copy  of  the  latest 
 version  or  complete  the  form  online  and  then  print. 


 If  you  have  not  obtained  your  FCC  Registration  Number  (FRN),  you  may  do  so  at  the  same  time  by 
 logging  on  to  our  FCC  website  at  http://  www.  fcc.  gov.  Click  on  "Commission  Registration  System" 
 located  on  the  left  side  of  the  page.
