 The  Wireline  Competition  Bureau  announces  release  of  a  revised  FCC  Form  499-  Q  (Telecommunications  Reporting  Worksheet)  that  carriers  must  file  to  report  their  revenues  from 
 the  prior  quarter  for  purposes  of  contributing  to  the  federal  universal  service  support  mechanisms.  1  The  revised  form  permits  affiliated  legal  entities  to  make  consolidated  filings  if 
 they  satisfy  and  certify  to  ten  conditions.  2  The  revised  form  has  been  approved  by  Office  of  Management  and  Budget  (OMB)  as  prescribed  by  the  Paperwork  Reduction  Act  of  1995.  3 


 Carriers  must  file  the  revised  FCC  Form  499-  Q  on  May  1,  2002.  Carriers  are  required  to  file  FCC  Form  499-  Q  reporting  their  revenues  from  the  prior  quarter,  by  the  beginning  of  the 
 second  month  of  the  current  quarter  (i.  e.,  February  1,  May  1,  August  1,  and  November  1).  4
