Pursuant to Section 76.403 of the Commission’s Rules, 47 C. F. R. §76.403, copies of Form 325 are mailed to cable television system operators for completion and return within 60 days after receipt. In accordance with the rules, copies of the form will be sent to all cable systems that serve 20,000 or more subscribers and to a random sample of systems with less than 20,000 subscribers. This form solicits basic operational information from system operators— including the operator’s name and address, system- wide capacity and frequency information, channel usage, and number of subscribers— and is intended to provide the Commission with core information that is needed to perform its regulatory functions. It was adopted in the Commission’s Report and Order in CS Docket No. 98- 61, 14 FCC Rcd 4720 (1999). Only system operators receiving a copy of the form through the mail and only those systems identified by the Physical System Identifier (PSID) on the form received are required to file it. In addition to the mailed copies, however, an electronic copy of the blank form in word processor format will be available from the Commission’s Internet web site at www. fcc. gov/ mb/ engineering/ Form 325. It is intended that the information, once collected and the data entered electronically, will be available to the public through the Commission’s Cable Operations and Licensing System (COALS) when that system is fully implemented.
