 The  effective  date  for  use  of  the  revised  Form  497  is  delayed  until  April  15,  2005.  Until  April  15,  2005,  eligible  telecommunications  carriers  seeking  reimbursement  for  low-  income  universal  service 
 support  must  use  the  old  Form  497.  The  old  Form  497  may  be  found  at  the  website  of  the  Universal  Service  Administrative  Company  at  http://  www.  universalservice.  org/  li/  forms/.
