Form 381 must be filed with the FCC no later than November 5, 2004. FCC Form 381 is to be used by full power TV broadcast licensees and permittees participating in the digital television channel election process to provide and certify the technical information that will be used to evaluate elections made during the channel election process. The electronic mailbox address is form381@ fcc. gov. If TV station licensees wish to file by U. S. mail, please address correspondence to: Federal Communications Commission, Media Bureau, Attn: Form 381 correspondence, Washington, DC 20554. Any correspondence filed in conjunction with the Form 381 will not affect a station’s service area that will be protected during the channel election process. That service area will be determined solely by reference to the certification in the Form 381 itself. Thus, when the Media Bureau issues a Public Notice after November 5, 2004 announcing a revised Table of Station Assignment and Service Information based on the Form 381 filings, any correspondence filed will not be reflected in that Table. In addition, any correspondence submitted will not necessarily excuse or affect the legal force and effect of the certifications made in Form 381. Nevertheless, in the exceptional case that a licensee or permittee wishes to describe special circumstances surrounding their Form 381 certifications, this provides a mechanism to do so.
