 The  Wireline  Competition  Bureau  announces  that  the  revised  FCC  Form  472  (Billed  Entity  Applicant  Reimbursement  Form  (BEAR))  and  the  revised  FCC  Form  474  (Service 
 Provider  Invoice  Form  (SPI))  and  instructions  have  been  approved  by  the  Office  of  Management  and  Budget  (OMB).  1  These  forms  are  to  be  used  by  applicants  and  /or  service  providers  seeking 
 reimbursement  for  discounts  under  the  schools  and  libraries  universal  service  support  mechanism.  Copies  of  the  forms  and  instructions  are  attached  to  this  document. 


 Copies  of  the  revised  FCC  Form  472  and  the  revised  FCC  Form  474  may  also  be  obtained  from  the  Universal  Service  Administration  Company  (USAC)  Schools  and  Libraries
