See OMB Control No. 3060- 0874. The revised Form 475 consolidates and streamlines data requirements making it less time consuming to complete and ensures that consumers have provided the minimum level of information required to process an informal complaint. Revised Form 475 significantly improves the informal complaint process for consumers, common carriers, and Commission staff.
