Applicants can avoid filing outdated editions by submitting their applications through the Universal Licensing System (ULS), given that changes to the paper forms will be incorporated into the system automatically. 1 To obtain a current copy of FCC Form 601 or FCC Form 605, visit the FCC forms page on the Web at http:// www. fcc. gov/ formpage. html or dial the FCC’s Fax Information System, 202- 418- 0177. You can also obtain paper copies of Form 601 or Form 605 from the FCC Forms Distribution Center by phone at 1- 800- 418- 3676. By way of background, on June 28, 2005, the Bureau announced changes to the ULS to accommodate the Broadband Radio Service (BRS) and Educational Broadband Service (EBS). 2 The Bureau also reminded licensees that use of ULS forms and electronic filing would become mandatory for BRS and EBS on July 11, 2005. As part of its continuing efforts to improve the license application process, WTB is finalizing revisions to FCC 601 Main Form, Application for Wireless Telecommunications Bureau Radio Service Authorization and Schedule B, for Geographically Licensed Services. In addition, a new Schedule E, and changes to Schedules D, I, and M, to be filed in conjunction with FCC 601 Main Form, are being developed. The new Schedule E is being developed for BRS/ EBS, and 1 ULS can be accessed at http:// wireless. fcc. gov/ uls. Schedule D is being changed, as well as Schedules I and M to implement changes to FCC rules governing areas known as “Quiet Zones.” At the same time, WTB is also making changes within ULS to accommodate a few minor changes to FCC Form 605, Quick Form Application for Authorization in the Ship, Aircraft, Amateur, Restricted and Commercial Operator, and General Mobile Radio Service. Effective with receipts of July 18, 2005, the Bureau will no longer accept FCC Form 605 applications that have an edition date prior to July 2005.
