C. 20554 News Media Information 202 / 418- 0500 Internet: http:// www. fcc. gov TTY: 1- 888- 835- 5322 DA 05- 2403 September 1, 2005 COMMISSION EXTENDS FORM 477 FILING DEADLINE FOR ENTITIES OPERATING IN ALABAMA, LOUISIANA AND MISSISSIPPI WC DOCKET NO. 04- 141 Form 477 requires certain entities (“ reporting entities”) to file this form on September 1, 2005. 1 Reporting entities include (1) facilities- based providers of broadband connections to end user locations, (2) providers of wired or fixed wireless local telephone services, and (3) providers of mobile telephony services. 2 Form 477 requires a reporting entity to file a form for each state in which the reporting entity operates and to provide specific information regarding that entity’s operations. 3 Due to the devastation caused by hurricane Katrina, President Bush issued a major disaster declaration for Louisiana, Mississippi, and Alabama on August 29, 2005. 4 Pursuant to our authority to waive rules for good cause 5 and to alleviate any additional burden that may be caused by our reporting requirements, we grant reporting entities that operate in those three states an additional 60 days, until October 31, 2005, to file the Form 477 for those states. In addition, to the extent that any reporting entity relies on personnel, facilities, or records located in one of those states to compile or file a Form 477 for a certain state, those reporting entities are also granted an additional 60 days, until October 31, 2005 to file the Form 477 for that state. 6 1 Form 477 Instructions § IV. We recognize that President Bush also issued a Major Disaster Declaration for the state of Florida on August 28, 2005. We do not extend the relief granted in this public notice to entities operating in the state of Florida, because the telecommunications infrastructure in Florida was not subject to the same level of destruction as was the infrastructure in parts of Louisiana, Mississippi, and Alabama. 5 See 47 C.
