 WIRELINE  COMPETITION  BUREAU  ANNOUNCES  DELAY  OF  EFFECTIVE  DATE  FOR  REVISED  FORM  497  USED  FOR  LOW-  INCOME  UNIVERSAL  SERVICE  SUPPORT

 On  October  4,  2004,  the  Wireline  Competition  Bureau  (Bureau)  stayed  the  use  of  the  new  version  of  Form  497  until  April  15,  2005.  1  The  Bureau  now  delays  the  effective  date  for  use  of  the  revised  Form 
 497  until  further  notice.  Eligible  telecommunications  carriers  seeking  reimbursement  for  low-  income  universal  service  support  must  continue  use  the  old  Form  497.  The  old  Form  497  may  be  found  at  the 
 website  of  the  Universal  Service  Administrative  Company  at  http://  www.  universalservice.  org/  li/  forms/.
