WIRELINE COMPETITION BUREAU ANNOUNCES OMB APPROVAL OF
THE REVISED 2007 FCC FORMS 499-A, 499-Q, AND ACCOMPANYING 
INSTRUCTIONS
On August 8, 2007, the Commission sought approval from the Office of Management 
and Budget (OMB) of the information collection for the revised FCC Form 499-A, FCC Form 
499-Q, and accompanying instructions. The revised forms and instructions incorporate changes 
required (1) by the decision of the U.S. Court of Appeals of the District of Columbia Circuit in 
Vonage Holdings Corp. v. FCC, and (2) a Commission order requiring interconnected voice over 
Internet Protocol (VoIP) providers to contribute to the Telecommunications Relay Service (TRS) 
fund.1  On September 11, 2007, the Commission received OMB approval.
