Line 26 of FCC Form 465 requires that participants identify the funding year for which they are seeking services. After a participant selects a vendor, the participant must file an FCC Form 466-A (Internet Service Funding Request and Certification Form) with a certification stating that all Pilot Program support will be used only for eligible Pilot Program purposes. Upon receipt and approval of a selected participant’s FCC Form 466-A and related attachments, USAC will issue a funding commitment letter for each Pilot Program funding year.
