The final drafts of the forms and instructions, to be used for informational purposes only, accompany this public notice. They will be available for actual use by E-rate participants after being posted on USAC’s website by January 11, 2011, before the opening of the 2011 application filing window.1 Applicants should NOT use the new forms until they are posted on USAC’s website. On that date, applicants will be able to file electronically or download a copy of the revised forms from USAC’s website, www.usac.org/sl. USAC will accept the previous FCC Forms 470 and 471, dated November 2004, for funding year 2011 and FCC Forms 470 that have already been filed do not need to be resubmitted. In addition, applicants that file the currently available Form 470, dated November 2004, will not have to have a technology plan for priority one services for funding year 2011, even though the certification at block 5, item number 21, indicates that such a technology plan is required.2 The Bureau submitted a revision of FCC Forms 470 and 471, collection 3060-0806, for OMB approval pursuant to 44 U.S.C. § 3507 of the Paperwork Reduction Act (PRA) of 1995.3 The Bureau sought comment on the proposed changes July 1, 2010.4 The revisions streamline the forms by eliminating questions that are no longer useful and limit data collection to information that is not already available elsewhere. Also, the FCC Form 471 now requires information about the current number of broadband connections and their speeds and requires that the Item 21 attachment be submitted with the form.
