In this order, we grant a petition filed by the Public Service Commission of the District of Columbia (DC PSC) on behalf of itself and the District Department of the Environment (DDOE) for waiver from 47 C.F.R. § 54.410(d) as amended by the Lifeline Reform Order.1 2. The DC PSC seeks a waiver until August 1, 2012 to comply with section 54.410(d) of the Commission’s rules requiring the state to provide prospective subscribers with a certification form that complies with the requirements of the Lifeline Reform Order.2 The DC PSC argues that DDOE, which is the state entity that is responsible for certifying and recertifying Lifeline subscribers, needs additional time to update the subscriber application and certification form to comply with section 54.410(d).3 The DC PSC argues that it needs additional time for the new form to be approved by the DC PSC, translated, printed and uploaded to the state database.4 As the DC PCS explains, the waiver it seeks would only apply to Lifeline subscribers for wireline services in the District of Columbia, because wireless Lifeline providers certify their own customers.5 3. During this waiver period, however, the DC PSC must still provide notice to the wireline ETC, Verizon Washington, DC Inc., that new subscribers meet the income or program-based eligibility requirements of the Lifeline program and have executed a certification form.6 4. ACCORDINGLY, IT IS ORDERED, pursuant to the authority contained in sections 1-4 and 254 of the Communications Act of 1934, as amended, 47 U.S.C. §§ 151-154, 254, and sections 0.91, 0.291, and 1.3 and 54.722(a) of the Commission’s rules, 47 C.F.R. §§ 0.91, 0.291, 1.3, that section 54.410(d) is WAIVED to the limited extent provided herein.
