Washington, D.C. 20554 News Media Information 202 / 418-0500 Internet: http://www.fcc.gov TTY: 1-888-835-5322 DA 13-1590 Released: July 17, 2013 WIRELINE COMPETITION BUREAU SEEKS COMMENT ON REVISIONS TO FCC FORMS 470 AND 471 CC Docket No. 02-6 Comment Date: August 16, 2013 Reply Comment Date: August 30, 2013 Pursuant to the Paperwork Reduction Act of 1995 (PRA),1 the Wireline Competition Bureau is revising existing collection 3060-0806.2 In this notice, we seek comment on proposed revisions to the Federal Communications Commission (FCC) Forms 470 and 471, which are used by applicants seeking support from the E-rate program (more formally known as the schools and libraries universal service support program). Copies of the proposed revised forms are attached to this notice. The purpose of the FCC Form 470 is to open a competitive bidding process for E-rate eligible services.3 After entering into agreements for services eligible for E-rate support, applicants file the FCC Form 471 to identify the services for which they will seek reimbursement from the E-rate program.4 Both forms include certifications to insure that applicants are complying with program rules.5 The proposed changes to the forms include: · Inquiring about broadband connectivity based on individual funding requests; · Requesting additional information about the current number of broadband connections, their type, and the speeds; · Consolidating telecommunications and Internet access services into one field on the FCC Form 470 so applicants are no longer required to specify whether they are seeking telecommunications or Internet access services; and · Adding a check box for “Federal Entity” applicants on the FCC Form 471. 1 See 44 U.S.C. § 3507 (setting forth requirements for an agency seeking to adopt or revise a public information collection). 2 See 60-Day PRA Notice for Collection 3060-0806, 78 FR 40475 (July 5, 2013).
