 On  October  7,  2002,  an  Order  in  CC  Docket  Nos.  96-  45  and  97-  21(  DA  02-  2543)  was  released  denying  Pajaro  Valley  Unified  School  District’s  Requests  for  Review  and  Waiver, 
 which  sought  review  of  two  decisions  issued  by  the  Schools  and  Libraries  Division  of  the  Universal  Service  Administrative  Company.  This  erratum  corrects  the  following  error  in  the 
 released  document: 


 In  the  heading,  replace  “Shepherdsville,  Kentucky”  with  “Watsonville,  California”  so  the  heading  shall  read:  “Pajaro  Valley  Unified  School  District, 
 Watsonville,  California”
