 By  the  Chief,  Telecommunications  Access  Policy  Division,  Wireline  Competition  Bureau: 
 On  August  13,  2004,  the  Fifth  Report  and  Order  and  Order  in  CC  Docket  No.  02-  6  (FCC  04-  190)  was  released  adopting  measures  to  protect  against  waste,  fraud,  and  abuse  in  the  administration  of  the 
 schools  and  libraries  universal  service  support  mechanism  (also  known  as  the  E-  rate  program).  In  particular,  the  Commission  resolved  a  number  of  issues  that  have  arisen  from  audit  activities  conducted  as 
 part  of  ongoing  oversight  over  the  administration  of  the  universal  service  fund,  and  the  Commission  addressed  programmatic  concerns  raised  by  our  Office  of  Inspector  General.  This  erratum  corrects  the 
 following  error  in  the  released  document: 
 On  page  41,  in  the  Appendix  B  Final  Rules  section,  the  first  sentence  of  paragraph  3  should  be  corrected  to  read  as  follows:  3.  Part  54,  Subpart  F  is  further  amended  by  adding  section  54.504(  h)  to 
 read  as  follows:
