 By  the  Chief,  Broadband  Division: 
 This  erratum  corrects  the  Public  Notice  released  September  24,  2004,  DA  04-  3060.  As  released,  the  Appendix  had  an  incorrect  title-“  APPENDIX-  GRANTED  WAIVER  REQUESTS.” 
 The  correct  title  should  be  “APPENDIX-  DISMISSED  PLEADINGS.” 
 This  action  is  taken  under  delegated  authority  pursuant  to  Sections  0.131  and  0.331  of  the  Commission’s  Rule,  47  C.  F.  R.  §§  0.131,  0.331.
