On July 30, 2015, the Wireline Competition Bureau released a Public Notice, DA 15-875, in the 
above captioned proceeding.  This Erratum amends the Public Notice as indicated below:
The entry beginning with “Summit Academy…” is removed from page 5 under the headline 
Invoice Deadline Extension Requests Less Than 12 Months Late, placed below “St. Peters School…” on 
page 6 under the headline Invoice Deadline Extension Requests More Than 12 Months Late, and corrected 
to read as follows:
