This Erratum amends the text of the Report and Order and Appendix A of the Report and Order as indicated below: 1. In paragraph 136, on page 65, the penultimate sentence is corrected to read as follows: “However, we require providers to update their election notice to include information on device type, device make, device model, and market value of the device.” 2. Paragraph (g) of Section 54.1810, on page 159, is corrected to read as follows: “(g) Restrictions on switching service offerings.
