 Implementation Date: On or after December 12, 2023

 Description of Planned Network Change(s):

North State Telephone, LLC dba Lumos, (“Lumos”) plans to retire the existing copper facilities
within t h e  Thomasville and Trinity areas of North Carolina, comprised of the locations listed in 
the impacted address attachment.  Lumos has existing fiber-to-the-home facilities for all of
the listed locations. At the completion of the transition all customers will be migrated from
copper to fiber facilities, and the copper will be retired.

 Description of reasonably foreseeable impact(s) of the planned change(s):

 After the retirement of copper facilities, copper facilities will no longer be available to
 serve customers at the attachedlocations and will not be maintained. However, all services currently
 provided over copper facilities will continue to be provisioned over fiber facilities.
